Rank: (The rank of the puppet. Genin and Chuunin can not make human puppets, or be made into puppets; Jounin and up only.)
History: (What happened? How did you kill him/her and end up making a puppet out of them? Explain in detail the person you've turned into a human puppet.)
Gadgets: (All the gadgets that have been implanted in the puppet. Explain both the purpose of the weapon and the location on the puppets body where the gadget is placed.)
Weapons: (If the person had any custom weapons that you kept, explain their abilities and the purpose they serve. Also, explain where on the puppet these weapons are kept.)
Jutsu: (Jutsu known before the persons death and are transfered to the puppet. Only a total of 6 jutsu can be transfered. These are permanent and cannot be changed, so think carefully.)
